Reminders For Your Appointment Day
Please bring the following:
- Your completed registration form. You may submit the completed form from our website in advance.
- Your dental insurance card(s). In order to provide you with flexible payment options, which include dental insurance coverage, we need the dental insurance card, cardholder’s full name, date of birth, social security number, and address at the time of your appointment.
- Your insurance referral, if applicable.
- The name, address, & phone number of your dentist.
- Review your dental insurance benefits.
